JOB PURPOSE:

* To act as a key member of staff responsible for the day to day
operation of Community Based Housing Service.
* To help ensure Galway Simon Community provides high quality,
effective housing support services.
* To work with a range of service providers in areas such as health,
welfare, justice, addiction services to ensure that clients have
access to all appropriate supports.
* To work with providers of training, educational occupational
services, to enable clients improve opportunities for social
integration and well-being.
* To assist clients living within community-based housing to find
appropriate pathways out of homelessness.
* To provide Tenancy Sustainment Supports to clients when they move
into independent living.

JOB PURPOSE: 

Housing Support Assistants work across our housing service, and
community support services working alongside our Housing Support
Officers, delivering practical and emotional supports to clients as
part of their overall support plan. The successful candidate will act
as a key member of the team, ensuring the day-to-day safe running of
the service, and in helping clients maximise their housing security
or, building connections in the community, supporting and preparing
them for move-on and independent living, as appropriate. This involves
delivering practical supports, the purpose of which will be to assist
people in areas such as health and well-being, self-care, personal
administration, and home management as necessary, using a trauma
informed approach.

JOB PURPOSE:

The successful candidate appointed to this position will work within
Homeless Services; working as part of multi-disciplinary team,
delivering a coordinated approach to client care and co-ordinating
services for particular cases as required. They will play a key role
within the Housing First Team, working to ensure that very vulnerable
clients can access the Health Supports that they need.

They will deliver and coordinate a broad-based integrated prevention,
education, health promotion and treatment nursing service and will
play a lead role on nursing issues within Homeless Services, to
include Housing First across the region. They will provide a service
for persons who are homeless or at risk of homelessness, to include
the delivery of advice and information as well as some training and
development. This service will be available to the homeless population
in Galway, Mayo and Roscommon. They will develop working relationships
with Primary Care Services throughout the Region to foster improved
access by the target group to health supports.
